We are looking to recruit a dynamic and forward thinking Clinical Matron to lead our recently transformed integrated Children’s Community Nursing Service.

Our established, passionate and dedicated CCN and Special School Nursing teams work collaboratively with children, young people (CYP) and families within the wider multi-disciplinary team to ensure care is provided within the community wherever possible. We provide high quality and holistic, family centred care to CYP with a nursing need across Portsmouth and East Hampshire area including those with;

  • Acute and short-term conditions
  • Long term conditions
  • Disabilities, complex conditions, those requiring continuing care and neonates

 

  • Life-limiting and life-threatening illness, including those requiring palliative and end of life care

Our team has recently undergone service transformation which has brought together three distinct elements of our service into one integrated team. If you are passionate about leading the ongoing development of Community Children's Nursing in order to support families to receive great care at or close to home, we would be delighted to hear from you.

You will be a registered Children's Community Nurse with experience of leading teams to deliver safe, effective, responsive, caring and well led health services within an integrated service framework. You will be able to demonstrate the ability to overcome barriers to facilitate seamless pathways of care across hosipital, community and primary care settings. You will be able to show a commitment to quality, evidencing engagement with service users, use of clinical outcomes and knowledge of evidence based clinical pathwaysin delivering community children's nursing services.

In this role you will have the opportunity to shape and lead our responsive service as it continually adapts to the ever changing needs of the families we care for. You will have lead care planning, training and multi-disciplinary liaison for CYP with a range of needs within their home, school and wider community settings.
