37.5 hours per week - you will be required to work a rota pattern which includes weekends and bank holidays between the hours 8am – 8pm

£22,477 - £29,323pro rata per annum

Care Plus Group is a Social Enterprise established since 2011 to deliver community healthcare services following the national separation of provider services from the commissioning arm of the local NHS. One of our core values is that staff and patients are integral to everything we do.  As part of this all staff employed by Care Plus Group automatically become members of the Group which means you will be able to give your views and vote on important issues for the organisation.  We welcome your application to join us.

The Community Nursing Service in North East Lincolnshire supports adults with on-going physical health needs who are housebound. Community Nursing staff liaise closely with GP’s, Practice Nurses and other agencies to provide a high quality, skilled, holistic nursing care service to the community.

We are looking to appoint a qualified registered level 1 nurse to join our experienced team based at Cleethorpes Primary Care Centre. You will initially work in conjunction with a named senior Community Nurse and will be responsible for the assessment of individuals’ care needs and the planning, implementation and ongoing evaluation of programmes of care.

You will have the ability to maintain clear and accurate records using information technology and will have a proven ability to work on your own initiative as well as part of a team. The post involves the day to day management of an unpredictable caseload, some of which will be complex in nature and therefore effective organisational and prioritisation skills are essential.

Most importantly you must be able to demonstrate your commitment to the values of Care Plus Group.

The post requires you to transport yourself and equipment across locations within North East Lincolnshire and to deadlines and you will be required to work evenings, weekends and bank holidays on a rota basis.  Therefore a flexible approach to your working arrangements is essential.

This is an exciting opportunity if you are looking for a career in Community Nursing as you will be fully mentored through your preceptorship and you will have the support of an established and experienced team.

For further information and an informal discussion please contact Karen Howes on 07775034141.
