Are you an ambitious, innovative and passionate Nurse, keen to use your leadership skills across Community Nursing in order to support the future development of teams across the West Locality?Being a positive role model, working with existing teams in order to deliver integration and ensure the continued safe delivery effective nursing care to patients and ensure effective use of resources Whilst working closely with senior colleagues to meet service requirements to reduce hospital admissions and deliver care for patients with complex health needs in their own homes? You will be based in the West Locality initially but potential within Southampton and work as part of the newly restructured Community Nursing Team, closely working with Case Management colleagues. You will work as part of the Senior Clinical Team, supporting and supervising team members and enabling them to provide high quality, person-centered care. Your excellent clinical leadership, interpersonal and team skills will enable you to work collaboratively across community, primary, secondary and social care settings.

As a Clinical Team Lead you will be responsible for the daily management of the team, holding overall responsibility for ensuring the team are providing safe and effective care and meeting the corporate objectives. You work closely with the Integrated Services Matrons and will be overall responsible for the operational and clinical delivery of care.

In return for your skills we offer regular clinical supervision with our both internal and external training opportunities and regular contact with other professional networks across the area. You will benefit from experienced and supportive colleagues and be able to develop your skills in a range of areas.
