Continuing Healthcare Assessment & End of Life Care

An exciting opportunity has arose for anyone passionate about providing high quality nursing support care. We are looking for a Healthcare Support worker (either full time and/or part time will be considered) who is compassionate and caring with the ability and passion to deliver high quality care which meets the needs of all our patients.

The individual will be enthusiastic, motivated and committed with the ability to work hard as part of a team. Sensitivity and empathy are also other key attributes. A basic level of IT is also required.

Jubilee House is a 25 bed adult inpatient unit situated in its own grounds in Portsmouth. The unit provides care for patients undergoing assessment of long term continuing healthcare needs and provides end of life care to those patients admitted in the final stages of their illness.

Jubilee House promotes a multi-disciplinary team approach, working to ensure that patients are (where appropriate) discharged to the most suitable environment which will continue to support individuals with their long term needs.

At Jubilee House, care is provided 24 hours a day, 365 days a year. As part of our commitment to provide consistent care, the post holder will be expected to be part of an internal shift rotation pattern as required, in order to meet the needs of the service.
