The purpose of the role is to support the general practice workforce and improve patient care. We are keen that this role develops in line with the national Clinical Pharmacist programme The post holder will be supported to complete the CPPE Training Pathway
Working as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
Supporting patients and the practice(s) with chronic disease management, specifically we are looking for a Clinical Pharmacist who can undertake respiratory reviews Asthma and COPD
Undertaking clinical medication reviews
Managing prescription processes and medication queries
Providing clinical leadership on medicines optimisation and quality improvement (including QOF)
Ensuring the integration of pharmacists across sectors
Development support and mentorship will be provided on a practice level, across the PCN and when
available on a locality/CCG level.

Demonstrates use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues, other professionals, other NHS / private organisations e.g. CCGs)

Actively work towards developing and maintaining effective working relationships both within and outside the practice and locality
Liaises with CCG colleagues including CCG pharmacists on prescribing related matters to ensure consistency of patient care and benefit
Liaises with CCG pharmacists and Heads of Medicines Management / Optimisation to benefit from peer support

Bristol Inner City Primary Care Network (BIC PCN) are looking to expand its Clinical and Operational teams, working at Charlotte Keel Medical Practice (CKMP) within our diverse and dynamic inner city Primary Care Network.
You will work as part of the BIC PCN, a forward thinking group of 7 surgeries comprising of Broadmead Medical Centre (BMC), Charlotte Keel Medical Practice (CKMP), Homeless Health Service (HHS), Eastville Medical Practice, Lawrence Hill Health Centre, Montpelier Health Centre and Wellspring Surgery.
