Working alongside adult social care, you will be responsible for the delivery of integrated, accessible and strengths-based interventions for adults with complex needs and substance use concerns.

You will need to focus on safeguarding, risk management and positive outcomes with the aim of reducing the need for costed adult social care packages.

Main duties of the job

Hold a caseload of clients with multiple needs. You will attend and contribute to multi agency meetings. You will also be required to update client records in a timely manner and complete formal reports. You will provide therapeutic and outreach support, including home visits and community-based work, and supporting access to work-related activities. You will work closely with social workers and multi-agency partners to carry out assessments, risk assessments, reviews and complete support plans, ensuring a person-centred approach.

Qualifications

Essential

  • A degree in Social Work, Psychology, Criminology, Health and Social Care or a related field is required due to the complexity of the role
  • Level 2 safeguarding trained, as a minimum
  • Good general level of education and evidence of continuing professional development

  • UK driving licence

  • Previous experience of working with adults with complex needs
  • Experience and knowledge of supporting vulnerable people and utilisation of structured needs / risk assessments
