GP – West Ipswich Care Management Service

Job summary
We are excited to share an opportunity to join the West Ipswich Care Management initiative which supports high users or predicted high users of acute non-elective beds. Care management is a way of intensively supporting this group of patients with proactive care co-ordination & additional GP and community matron capacity.
We have started this service in the West Ipswich INT area and our aims are:
- To improve the care of these complex patients
- Ease workload pressures across the system
- Reduce hospital admissions
We are now seeking experienced GPs to work as part of this multi-disciplinary team. You'll attend a daily MDT meeting at a practice site to thoroughly discuss the care plan for the relevant patients, before carrying out rapid response appointments and home visits as necessary.
Whilst you will be an experienced GP, with a likely interest in frailty, multimorbidity and proactive care planning, you will utilise your skills outside of the typical general practice set up.
We are looking for individuals who thrive when working across organisational boundaries to deliver the best care.
Contracting arrangements:
We can be flexible to your pattern of work, responsibilities and preferences:
- Self-employed or PAYE bank ad-hoc sessions
- Regular contracted sessions
- Sessions run Monday Friday (full day 7.5 hours)
- Session rate set at £70 per hour (plus PAYE benefits) or £78.38 self employed

Main duties of the job
- Fully qualified GP with GMC registration
- Annual appraisal and revalidation (when appropriate)
- Be on the NHSE medical performers list
- MRCGP
- Vocational Training Certificate or equivalent JCPTGP
- Certificate of Completion of Training (CCT)
