Job summary

Stanford Le Hope and Corringham PCN has an exciting opportunity for a permanent social prescriber to join our practice team. We are recruiting for full and part-time roles and can be flexible with working week patterns.

Social prescribing empowers people to take control of their health and well-being through referral to non-medical link workers who give time to focus on what matters to each individual, taking a holistic approach and connecting people to community groups and statutory services, for practical and emotional support.

You will also work closely with the integrated locality team, local services and Thurrock council to refer patients where needed and have a huge support network to assist you in your role. Main duties of the job

  • Working on PCN projects to reduce hospital admissions and raise awareness.
  • Providing information and advice about a wide range of services in order to promote health and well-being in order to support independence within communities.
  • Forging strong links with local VCSE organisations, community and neighbourhood level groups.
  • Contributing to reducing demand on statutory services, hospital admissions and to combat unnecessary GP appointments.
  • Working with patients in the practice, or with patients in their own homes, to provide personalised person-centred support.

About us

Stanford Le-hope and Corringham PCN works with 6 surgeries in the local area to improve capacity and access and to help delivery better clinical care for its patients.

Our PCN has its own estate so you will be working within a busy friendly and experienced clinical environment, where you will be part of our team.

  • NVQ level 2 or equivalent in literacy
  • Diploma Level 2 in Health and Social Care, or equivalent qualification or experience.

Experience

  • Experience of working within healthcare, the voluntary or community sectors, supporting vulnerable groups.
  • Strong listening and communication skills.
  • You will be familiar with local resources and services and how to access them.
  • Motivated by helping people, with care and empathy.
