Job summary
Taunton Dean West PCN (Wellington and Wiveliscombe) are looking for a full time administrative care coordinator to join their team.
The Administrative Care Coordinator plays a key role in delivering high-quality administrative and personalised care support across Taunton Deane West Primary Care Network (PCN). Working across Lister House and Wellington Medical Centre, the post holder ensures the smooth organisation of MDT meetings, proactive coordination of care for patients and efficient completion of core practice administrative tasks, including appointment booking, summarising communication handling and accurate record keeping.
They will work closely with GPs, MDT members, Social Prescribing Link Workers and Health & Wellbeing Coaches to support the delivery of the PCN DES, ensuring patients receive personalised, holistic care. This includes shared decision-making, care navigation, population-health-driven case finding and support planning for individuals with complex needs, long-term conditions, frailty, cancer or those living in care homes.
The role is essential in reducing health inequalities, enhancing patient experience and ensuring seamless coordination across services, acting as a reliable point of contact for patients, carers and professionals.

- Coordinate and provide cover for MDT meetings, including preparing agendas, gathering information, attending meetings, producing accurate minutes and maintaining action logs.
- Use clinical systems and population-health tools to identify patients requiring proactive coordination and support.
- Support with Practice and PCN administration such as book appointments, manage enquiries, process documents, referrals and administrative workflows, ensuring accurate and timely updates to patient records.
- Support shared decision-making and personalised care and support planning, working closely with Social Prescribing Link Workers and Health & Wellbeing Coaches.
- Provide high-quality care navigation, helping patients understand care options and access community, social and self-management resources.
- Maintain communication across primary care, community, social care and voluntary services to ensure smooth transitions and holistic support.
- Uphold governance, confidentiality, CQC requirements and safe working practices, contributing to public health campaigns and high-quality patient care.
