Job summary
A Registered Nurse working across residential rehabilitation and step-down services provides specialist, recovery-focused care to adults with complex mental health needs, complex emotional needs, and associated social, behavioural, and physical health challenges. The role is centred on supporting individuals to achieve their personal recovery goals, maximise independence, develop coping strategies, and build the skills and confidence required to live successfully within the community.
Working within a trauma-informed and person-centred framework, the nurse undertakes comprehensive mental and physical health assessments, develops and reviews care plans, and delivers evidence-based interventions that promote wellbeing, resilience, and social inclusion. They are responsible for the safe administration and monitoring of medication, identifying changes in mental state, managing risk, and responding effectively to crises, self-harm, emotional dysregulation, and behaviours that may challenge services.
The post holder works collaboratively with residents, families, carers, and multidisciplinary professionals to ensure care is coordinated, responsive, and tailored to individual needs. This includes supporting individuals to access community resources, develop daily living skills, maintain meaningful activities, and prepare for progression through rehabilitation pathways and eventual transition to more independent living arrangements.
Main duties of the job
As a key member of the clinical team, the Registered Nurse provides leadership and professional guidance to support staff, promotes therapeutic engagement, safeguards vulnerable adults, and contributes to maintaining a safe, positive, and recovery-oriented environment. The role requires strong clinical judgement, excellent communication and risk management skills, and the ability to build trusting relationships with individuals who may have experienced significant trauma, long-term mental ill health, or repeated service involvement.
The post holder enables individuals with complex mental health and emotional needs to maximise their independence, improve their overall wellbeing and quality of life, and work towards meaningful, person-centred recovery goals. Through the provision of therapeutic, recovery-focused, and trauma-informed care, they support individuals to develop coping strategies, build resilience, enhance daily living skills, and strengthen social and community connections. Working collaboratively with multidisciplinary teams, families, and external agencies, the post holder promotes positive risk-taking, supports successful transitions through rehabilitation and step-down pathways, and empowers individuals to achieve sustainable recovery and live as independently as possible within their communities.

require support through rehabilitation and crisis, supporting a multi-disciplinary approach that

They will provide professional day to day leadership to the multi-disciplinary team and associated
staff within the specific area of service. This role involves multidisciplinary and multiagency
working. It requires specialist knowledge evidenced by the relevant post graduate study, with
high levels of professional wisdom, confidence, and maturity.
Protects, promotes and monitors the safety and welfare of individuals and their carers by
holistically assessing individual needs and developing and designing care and intervention
plans, treating each person as a unique individual. Wherever possible they will promote the
individuals views and wishes in order to be able to support the individuals rights, by adopting a
least restrictive approach.
Applies specialist knowledge and analytical skills in supporting service users to make informed
decisions to improve the quality of their lives by minimising the impact of their mental illness and
psychosocial stressors on their life. Safeguarding vulnerable people, promoting recovery focused
interventions, which is often in highly emotive situations where there are high levels of complexity,
uncertainty, crisis and risk.
Provides a clinical and social perspective to the causes of islanders poor mental health, and
where appropriate, promotes and enables the use of alternatives to hospital admission.
Works collaboratively with colleagues to empower service users and carers, maximise service
user independence, and ensure the protection of islanders human rights
Takes responsibility for continuing professional development within own area of expertise, with
particular regard to the relevant legislation, and the safeguarding of adults and children.
Provides timely, accurate, well written reports, in accordance with statutory guidance and
Government policy.
Provides professional advice, guidance, and training to others as required.
The practitioner performs their work role in a multi-disciplinary, multi-agency context. It is
essential that they have a detailed knowledge of the broad range of professionals and resources
they may need to call on, and that they maintain healthy relationships with individuals within
these services.

communicating in an appropriate, open, accurate, transparent, and straightforward way. Clearly
