Job summary

The postholder will deliver and develop a specialist service for a group of patients with a defined set of conditions involving technical interventions and the provision of advice within a multi-disciplinary framework.

This will involve developing and maintaining a high clinical profile and presence and acting as role model for other nursing staff.

Main duties of the job

The post holder will work as part of a dedicated team carrying out statutory duties within a home treatment team, using key skills to effectively carry out comprehensive mental health and risk assessments. Being able to appropriately support service users as to manage risk and meet identified needs. Incorporating carers and significant others in the individuals care.

To provide a resource of specialised mental health intervention, face to face to people who require support through a crisis, providing a multi-disciplinary approach that best suits the clinical need of the service user.

To offer increased and additional input as an alternative to an in-patient admission.

To provide professional day to day leadership to the multi-disciplinary team and associated staff within the specific area of service. This is an autonomous and authoritative role, involving multidisciplinary and multiagency working. It requires specialist knowledge evidenced by the relevant post graduate study, with high levels of professional wisdom, confidence, and maturity

Applies specialist knowledge and analytical skills in supporting service users to make informed decisions to improve the quality of their lives by minimising the impact of their mental illness and psychosocial stressors on their life. To safeguard the lives of vulnerable people, promote recovery focused interventions, which is often in highly emotive situations where there are high levels of complexity, uncertainty, crisis and risk.

Provides a social perspective with regard to the causes of islanders poor mental health, and where appropriate, promotes and enables the use of alternatives to hospital admission.

Works collaboratively with colleagues to empower service users and carers, maximise service user independence, and ensure the protection of islanders human rights.

Takes responsibility for continuing professional development within own area of expertise, with particular regard to the relevant legislation, and the safeguarding of adults and children.

Provides timely, accurate, well written reports, in accordance with statutory guidance and Government policy.

Provides professional advice, guidance, and training to others as required.

The senior practitioners perform their work role in a multi-disciplinary, multi-agency context. It is essential that they have a detailed knowledge of the broad range of professionals and resources they may need to call on, and that they maintain healthy relationships with senior individuals within these services.

To lead establishing and maintaining the trust and confidence of individuals and their carers by communicating in an appropriate, open, accurate, transparent, and straightforward way. Clearly explaining service policies and the professional role and responsibilities of the multi-disciplinary team.
