Job summary
An opportunity has arisen within the Community Drug and Alcohol team for a Community Substance Misuse Worker to join a developing substance misuse team within the Ceredigion locality, based in Aberystwyth.
We are looking for an individual who has experience of working with substance misuse and can demonstrate core and specific competencies related to tier 3 substance misuse work or who is keen to develop their skills and experience within a tier 3 substance misuse service.
Experience in the assessment and treatment of service users with opiate dependency would be an advantage as well as a good working knowledge of harm reduction approaches and managing prescribing regimes within a recovery focused treatment framework.
Main duties of the job
The post holder will act as care manager for the caseload and any significant change in circumstances that may result in increased risk to the service user or their significant others must be discussed in the appropriate forum. This will include:
Comprehensive assessment of the service user's substance misuse problem including their health and wellbeing needs and level of motivation to change.
Agreeing a care plan /aftercare / discharge plan with the service user that may include other professionals or onward referral as appropriate.
Implementation and review of the care plan and associated evaluation in line with service and Health Board policy.
Consideration of the needs of significant others and the opportunity to engage them in the care plan.
Joint working with partner agencies and contributing the treatment and clinical components of the agreed care plan where the service user is subject to a treatment order through the courts.
