An exciting opportunity has arisen in North Wales GP Out of Hours for Band 8A Advanced Health Care Professionals to join our forward thinking team to provide a high standard of clinical care to the North Wales population.
We are looking for individuals who are outstanding in their field and can bring enthusiasm and motivation to the role as a leader for the service. We recognise the importance of having Advanced Nurse Practitioners in our team and are looking to introduce this role to help us grow and expand our service.
Applicants must have a Masters degree with an Independent prescribing qualification. We will expect you to provide same day care to patients presenting with new onset symptoms and/or acute exacerbations of diagnosed long-term conditions from three months old. You will undertake visits to patients in their own home, community hospitals or care homes and undertake verification of death. You will support the GP Out of Hours team by providing knowledge, expertise and leadership.
We offer flexible working patterns to meet individual needs based in West, Central and East depending on where you prefer to work. You will be paid enhancements on all hours worked in the Out of Hours period.
GP Out of Hours has a supportive management team who are available to assist and support you during your working hours. Although we work in different bases, we work as a team to ensure we provide a supportive environment.
