Job responsibilities

The Cambridgeshire Learning Disability Partnership (LDP) bringstogether specialist health and social care services for people with alearning disability.

We have two exciting Social Worker vacancies inourSouth Cambs Team, based at Sunley House, Papworth Everard.

The successful candidates will contribute to thisunique model of practice as part of an integrated team alongside Health and Social Care Practitioners such as Adult Support Co-ordinators, Social Workers, Community Nurses, Occupational Therapists, Physiotherapists, Psychologists, Psychiatrist, Speech and Language Therapists and Art / Music Therapists.

The LDP are currently working to a hybrid model in line with our service needs. Following an induction period, our Social Workers will work from the office up to three days each week. This gives our Social Workers a quiet space and plenty of time for reflection whilst still balancing collaborative working across the multi-disciplinary team and peer support. It has also increased our Social Workers autonomy and has had a positive impact on wellbeing and work-life balance.

Cambridgeshire County Council has a collaborative approach to social work and social care to make it increasingly proactive, preventative, and personalised. Our staff work with individuals, carers, and families considering holistic, creative solutions to enable individuals to meet their aspirations through the care and support planning process.

What will you be doing?

You will be undertaking Social Care and Mental Capacity Act assessments, completing Safeguarding enquires as well as care and support planning, to achieve better outcomes for the adults we support. You'll need to be inquisitive and provide evidence based, proportionate recording with a keen focus on strength-based practice.

As youll be working with adults, both those in need of care and support services, along with their informal carers, it will be crucial that you work in partnership with them to establish needs and ensure that they are met.

Inquisitive and creative practice is embraced, to allow for personalised solutions, considering wider community resources and organisations. This isnt just about delivering care, its about providing service users with choice and control of their lives and empowering independence.

About you

We are looking for people who can demonstrate excellent written and verbal communication skills, have the ability to work both independently and as part of a multi-disciplinary team, and are committed to making a positive difference for the people we support. The successful candidates will have excellent organisational skills andbe willing to use their initiative to work in innovative ways.

You must:

  • Be aqualified Social Worker with a Social Work England registration
  • Canwork from a variety of locations and travel countywide independently
