Job summary
We have an exciting opportunity for a Occupational Therapist at our Primary Care Network (PCN),The One Network Alliance Ltd is a Primary Care Network (PCN) covering the Bethnal Green locality. PCNs bring together neighbouring GP practices and partner organisations to deliver joined-up, high-quality care for their registered populations.
The post holder will work in partnership with service users to develop personalised recovery and support plans. The role will primarily support patients on the complex care caseload, many of whom are living with significant frailty, multiple long-term conditions and may be housebound. The focus will be on assessing individuals needs and identifying appropriate interventions to maintain independence and improve quality of life.
The post holder will work across the PCN footprint, delivering sessions from member practices and undertaking home visits where required. They will represent and promote the Occupational Therapy service across the PCN, working collaboratively with practice and wider PCN teams while also managing their work confidently and autonomously.
The post holder will work closely with, and report to, the Senior Occupational Therapist.
This is a part time role - 22.5 hours per week.

Clinical
- To apply a high level of understanding of the effects of disability on
occupational performance and provide training and advice on lifestyles changes
and adaptations to the patients social and physical environment
- To have a high level of understanding of the effects of dementia and to
undertake appropriate assessments and interventions helping those with dementia
to retain existing function and to be able to safely live within their own home
for as long as possible
- To assess patients capacity, gain valid informed consent to treatment
and where such capacity is lacking/absent to work within Mental Capacity Act
and associated legal frameworks in the management of the patient
- To promote social inclusion and physical and mental well-being using
evidence- based practice. This is underpinned by the principles of hope and
recovery, health promotion and relapse prevention
- To independently manage a clinical caseload of service user with complex
