The NHS Place Director for York is seeking to appoint a Senior Programme Manager support and embed the collaborative relationships across York Health and Care Partnership, including driving the progress of the partnership priority projects, and the adoption and spread of Population Health Management Tools and Techniques across the system.

This leadership role is situated at the heart of the York place system; where NHS and Local Authority comes together with communities and partnerships.

Main duties of the job

This post will work within Humber and North Yorkshire ICB and to the broader partnership of the York Health and Care Partnership to support the progress of the partnership priority projects, and the adoption and spread of Population Health Management Tools and Techniques across the system.

The role involves working strategically to support the Partnership Board and Leadership team in delivering set priorities as part of its emerging role as the Place Based Partnership for York within the North Yorkshire and Humber Integrated Care System.

These priorities are being built around population health data, and this role exists to turn this insight and partner priorities into reality, 'doing' real projects and providing effective advice and hands on support to local teams and partners.

The role will also co-ordinate various aspects of planning across the place partnership and NHS system including place priorities, the place aspect of the ICB's Joint Forward Plan, and NHS operational planning.

About us

It is a leadership role, tasked with ensuring integrative approaches between health partners and between health and social care are taken, to improve quality of care, empower clinicians, engage seldom-heard communities and deliver population health management in our place. The role supports two key programmes of work:

Programme #1 York Health and Care Partnership

Our York Health and Care Partnership is an exemplar of the way place-based partnerships under a larger Integrated Care System are starting to function. Drawing from a cross-section of leaders in health and care, we are asking people work together in a new way, shaping services and a new culture within York which will lead to integrated services and more joined-up care. This role will offer senior programme management resource for the York Partnership, working closely with CCG and council executives, our facilitator and other partners.

Programme #2 York Population Health Hub

Under the recently established Population Health Hub for York, a virtual team of data analysts, service improvement managers and public health specialists, are working together to embed a population health approach and population health management techniques into our work as a health and care system. The work centres around three pillars:

Enabling: training, developing capacity and supporting the York health and care system to use population health data, and PHM as a tool

Analysing: delivering the JSNA, making it useful and used

Doing: tangible, real work leading PHM projects which show the benefit of this approach

The post holder will offer senior programme management resource for the Hub, and provide the capacity we need to deliver its objectives, whilst the Director of Public Health for City of York Council provides specialist and technical population health expertise into this work and is the Senior Responsible Officer for the project.

As a Senior Manager, you will also be required to work closely with colleagues at Place and System Level in supporting the delivery of operational and strategic goals. You will be expected to act as a team-player and support your colleagues, to develop and progress against the teams deliverables.

Specific requirements of the role include:

Working effectively as part of a wider team to deliver a successful Place-Based Partnership in York and support a range of products and supporting materials to help local partners

Influencing without formal authority, and taking an autonomous approach to work

Cajoling senior leads across the system to drive forward progress on population health management

Understanding of Population Health and prevention priorities, and comfortable with discussing and presenting highly complex data and health intelligence

Engaging, listening, leaning and co-designing the Alliance and population hub with partners

Dealing well with uncertainty, on new projects with significant possibility of changing goalposts and new tasks 'thrown at you' but with real scope for finding yourself at the heart of an innovative 'place-based' commissioning model.

Work closely with the AD for System Planning and Improvement to co-ordinate an approach to planning that clearly links the priorities of York Health and Care Partnership with NHS operational planning, working with place and system colleagues to ensure all partner organisations are engaged in development of the partnership and place plans.

A key expectation of the role is that you will be able to operate as a network with your colleagues who represent other Places across the system.

In the role of Senior Manager you will provide senior input to the delivery of the team objectives and manage and line manage support team members to ensure an effective support offer to systems.
