Job summary

Frimley has an ambitious plan to address the priorities for people with learning disabilities and autism in the NHS Long Term Plan, and work with partners to make Frimley a place where people are supported to live long, healthy, and fulfilled lives. Promoting a strengths-based culture and proactively addressing any deficits-based perspectives, this includes active work to improve treatment and support for inpatients, a range of developments to strengthen multiagency community provision to prevent unnecessary admission to hospital, action to address health inequalities for both children and adults, and assessment, diagnosis and post-diagnostic support for children and adults with learning disabilities, autism and ADHD. This programme of work is supported and assured by NHSE through a range of regular reporting, meetings and deep dives.

Main duties of the job

The Dynamic Support Hub Practitioner will be an innovative manager, playing an important role in preventing unnecessary admission to hospital of children and adults with a learning disability or who are autistic and ensuring those who require admission to hospital receive safe and effective treatment and timely discharge. They will be responsible for supporting the CYP Complex Needs Liaison Nurse with the ongoing development and delivery of the Dynamic Support Hub in East Berkshire and maintaining oversight of dynamic support and patient assurance for the whole of NHS Frimley Integrated Care Board area. This includes managing a Dynamic Support Register supported by processes and expertise to ensure children and adults who have a learning disability or autism and behaviours that challenge are systematically identified and receive proactive preventative support when their needs are escalating. The right candidate will be a qualified RNLD or RMHN with experience of the Transforming Care Programme and multiagency working.

Frimley ICB incorporates East Berkshire (Slough, Windsor & Maidenhead and Bracknell Forest), Surrey Heath, Farnham and North-East Hampshire. Close partnership working is key to this role, particularly with neighbouring ICBs (Hampshire/ Isle of Wight and Surrey Heartlands) and the Provider Collaboratives who oversee Frimley patients in secure settings and children/ young people in residential and inpatient settings, as well as local authorities, health, education and the voluntary sector

A key part of this role will be undertaking 8-weekly Commissioner Oversight visits to adult inpatient services, both our local service in Reading but also other areas of England, so a willingness to travel to conduct these visits is essential. Most other aspects of this role can be successfully achieved via remote working, though Frimley ICB have office bases in Aldershot, Camberley and Windsor, with bookable desks and meeting rooms. There will be occasions when the post holder will need to visit areas that form Frimley ICB for in-person events such as away days.

Knowledge, Training and Experience

Essential

  • Professional nursing qualification (RNLD or RMN) with experience of working with people who have learning disabilities and autism.
  • Knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training.
  • Evidence of post qualifying and continuing professional development.
  • In depth knowledge of the Transforming Care agenda and priorities.
  • Experience of support planning with children or adults who have a learning disability or autism and behaviours that challenge.
  • Partnership working across a variety of NHS and Social Care settings and with families and service users.
  • Communicating clearly to variety of audiences through written material, presentations, and conversation
  • Experience of developing effective processes and procedures through collaboration
  • Member of relevant professional body
