Practice Pharmacist, Band 7 - Warrington NHS Cheshire and Merseyside
One position - part time 22.5 hours
We have an exciting opportunity for a practice pharmacist to join the Warrington Medicines Management Team. The position is to support, promote and implement high quality, evidence-based, cost-effective and safe use of medicines in across our locality. The post holder would be working as part of the medicines management team to support in optimising patients' medications, improving quality and reduce risks associated with medicines.
Main duties of the job
This role will directly support the Senior Pharmacist and Senior Medicines Optimisation Lead with regards to safe, high quality and cost-effective prescribing, including delivery of the Warrington Medicines work plan.
The applicant must be a registered GPhC pharmacist with the ability to deliver medicines management and optimisation services in GP practices. You will work with our team ofmedicines management pharmacists and technicians to provide medicines optimisation support to GP practices across Warrington.
You should demonstrate excellent clinical skills and must be computer literate. You must be organised, be able to work independently and also be an excellent team player.

This includes patient medication reviews, helping practices achieve prescribing targets and supporting the implementation of national and local guidance in order to improve patient care.
The postholder will act as a specialist resource to GP Practices, to answer medicines-related queries, to signpost to other services and to liaise with other healthcare professionals, including secondary care when required.
The postholder will contribute to projects across the ICB footprint, as required by the Senior Medicines Optimisation Lead.
