Job summary

Clinical and care professional leadership is central to decision-making and delivery for the Lewisham Health and Care Partnership and we are recruiting to a further role focused on Older Adults (0.2 wte)

We are looking for health and care professionals to lead and champion improvements in care across organisational boundaries, and tackle inequalities in our population through closer collaboration. We want to see a leadership cohort that is diverse in their experience, knowledge and background, and reflective of the diversity of the communities we serve.

Main duties of the job

The core clinical and care professional leadership team will work closely with the Lewisham place SEL ICB team to define and deliver agreed workplans, with health inequalities, quality, patient experience and safeguarding being integral to all areas, while a population health management approach and data should inform all service change and transformation.

CCPLs will be expected to work as part of multidisciplinary teams and support the delivery of both system and place priorities with particular emphasis on cross-boundary integration and delegation wherever appropriate of decision making to places and neighbourhoods. All postholders will be expected to understand and address the challenges of health inequalities and the principles of

population health management and quality to ensure evidenced-based optimisation of care and services where they are most needed.

The pay range for those roles at 0.2 wte is £18,540-22,660. Please do not apply for these roles if the salary does not fit with your salary expectations.
