Are you a registered
nurse looking for a role in District Nursing. We are looking for an experienced
and motivated individual who will be responsible for carrying out holistic and
autonomous evidenced-based nursing care to patients under the care of the
Warrington Out of Hours District Nursing service. You should be able to
demonstrate ongoing professional development.
The post holder will
provide a high standard of care, utilising clinical skills in the assessment,
planning and implementation of pro-active care to a community population with
nursing needs referred into the district nursing service. The post holder will
work in close collaboration with other professionals/stakeholders within the Warrington
footprint.
If you are looking for
flexibility with your working hours to support your work life balance, all
flexible working requests will be considered in line with the needs of the
service.
Main duties of the job
The OOH role focuses on promoting patient safety, health
and wellbeing through timely assessment and intervention in response to urgent
and priority clinical needs. The postholder undertakes comprehensive
assessments of Priority 1 callouts, identifies changing or deteriorating health
needs, delivers appropriate nursing interventions and evaluates outcomes. They
work autonomously and without direct supervision, demonstrating clinical
competence, professional accountability and adherence to NMC standards, while
maintaining accurate and contemporaneous records.
The postholder works collaboratively with the wider
multidisciplinary team, patients, relatives, carers and relevant health and
social care services to ensure safe and effective care. The role does not
involve holding a caseload or undertaking planned chronic disease management;
instead, it is focused on responding to urgent and unpredictable OOH demand,
prioritising patients according to clinical need, managing potentially complex
or challenging situations, and escalating concerns appropriately. Effective communication,
sound clinical judgement and the ability to work independently are essential to
maintaining patient safety and continuity of care across the OOH service.
