Job summary

Are you a motivated and compassionate Registered Nurse looking to develop your career in community nursing? We are seeking enthusiastic Band 5 Staff Nurses to join our dynamic Warrington District Nursing Team.

In this role, you will deliver high-quality, person-centred care to patients in their own homes and community settings. Working under the supervision of the District Nurse, you will manage a delegated caseload, undertake comprehensive assessments, plan and evaluate care, and respond to changing health needs. You will build skills in key areas such as wound care, palliative and end-of-life care, chronic disease management, and catheter care, while maintaining accurate records in line with NMC standards.

Collaboration is central to the role, and you will work closely with the multidisciplinary team, social care, and voluntary organisations to ensure coordinated, holistic care. Strong communication skills are essential when supporting patients, families, and carers, often in sensitive or complex situations. You will also contribute to the development of junior staff and students through mentoring and teaching.

This role offers autonomy, variety, and excellent opportunities for professional development, with support from experienced District Nurses and Team Leaders.

Main duties of the job

The post holder will play an essential role within the District Nursing Team by delivering high-quality, patient-centred care under the supervision of the District Nurse. They will undertake comprehensive assessments of patients nursing needs, develop and implement individualised care plans, and continuously evaluate the effectiveness of care, ensuring that all interventions respond to patients changing health needs. Close liaison with the District Nurse is expected to ensure seamless communication, continuity of care, and ongoing review of complex or continuing care packages.

The role involves regular communication and collaboration with a wide range of professionals and agencies, including members of the Primary Health Care Team, voluntary organisations, and statutory services. This multidisciplinary approach ensures coordinated, holistic care for patients and supports the wider health and wellbeing of individuals, families, and the community. The post holder will maintain accurate and contemporaneous records in line with NMC requirements and may attend multidisciplinary case conferences as the named nurse when appropriate.
