** THERE ARE 2 ROLES AVAILABLE;

ROLE 1 - ASSISTANT PRACTITIONER working with the team DOMICILIARY THERAPIES WITHIN MERTON LOCALITIES (27 hours and 45 minutes per week)

ROLE 2 - ASSISTANT PRACTITIONER working with the MERTON ENHANCED HOME BASED THERAPY TEAM (23 hours and 35 minutes per week)

PLEASE NOTE - This role is not eligible for sponsorship - as the role is part time and fixed term.

Main duties of the job

o Assisting in the identification of health and social care needs and participating in the delivery and evaluation of care packages developed to meet those needs under the indirect supervision of a registered health professional.o Supporting Occupational Therapists in the safe and effective management of delegated caseloads, delivering interventions under supervision and working within their scope of practice, recognising professional limitations and escalating concerns appropriately.o Working as part of an interdisciplinary team within community rehabilitation services to support the development and maintenance of effective working relationships with statutory, voluntary, and independent organisations.o Developing and maintaining clinical skills and competencies through active involvement in the community rehabilitation functions of the service.o Managing a delegated clinical caseload comprising adult patients with physical and cognitive difficulties within community settings, in accordance with service protocols and individual competencies.o Collaborating with colleagues across community teams, acute services, General Practitioners (GPs), social services, and voluntary sector organisations to optimise the patient journey, enhance wellbeing, and improve patient outcomes.o To carry out basic functional assessment to enable a further trusted assessmento Act within the scope of practice & to recognize boundaries of own practice and responsibility.

Essential

Educated to Foundation Degree in Care
Higher Professional Diploma in Health and Wellbeing (NVQ / QCF - Level 4) or equivalent
NVQ/QSF level 3
A recognised equivalent course in health and/or social care validated by the Regulated Qualification Framework (RQF) or a Level 3 or higher course in a health and/or social care subject from a Higher Education Institute
Equivalent Combination of NVQ/QSF level 3 or higher short courses and experience. e.g. as a Therapy Assistant, Health Care Assistant, or equivalent

Desirable

GCSE grade 9 - 4 or equivalent in English and Maths
Willingness to undertake further training as appropriate
Formal education / qualification in exercise

Experience

Essential

Knowledge of NHS
Experience of working within a healthcare environment.
Experience of working within community
Experience of working as part of a team
Experience of working with / managing patients with frailty or other long term conditions.
Demonstrable employment history working with a healthcare environment as part of a team

Rehabilitation experience
Previous experience working with Therapies within health or social care

Good knowledge of therapeutic moving and handling skills
Able to carry out basic therapeutic techniques as delegated by a qualified member of staff.

To be able to instruct patients and carers on a range of physiotherapy and mobility activities
Able to keep legible and accurate patient information and records
Understanding of the roles of the members of the multidisciplinary team
Can demonstrate ability to learn new procedures and techniques and follow instructions
