We're seeking a dedicated senior nurse to join our dynamic team and contribute to our specialised Continence Service for people living in the London borough of Merton .

We work collaboratively with a multidisciplinary team of healthcare professionals to deliver personalised treatment plans and support services to meet the unique needs of each patient.

Main duties of the job

  • Lead and coordinate the delivery of nursing care within the Continence Service ensuring high standards of clinical practice and patient safety.
  • Conduct comprehensive assessments, develop individualised care plans, and provide evidence-based interventions for patients with bladder and bowel conditions.
  • Collaborate with healthcare professionals, including physicians, therapists, and support staff, to optimise patient outcomes and continuity of care.
  • Provide education and support to patients and their families regarding bladder and bowel management techniques, lifestyle modifications, and self-care.
  • Participate in quality improvement initiatives, research projects, and professional development activities to enhance the effectiveness and efficiency of the Bladder and Bowel Service.

Essential

  • Registered Nurse with current UK NMC registration, Diploma/degree in Nursing
  • Mentorship qualification
  • Post graduate diploma/degree in specialist nursing area relevant to the post.
  • Non-medical Prescriber (V100/150/200 minimum)
  • Management training certificate or in-house first-line management training course or equivalent experience managing a nursing team.
  • Evidence of continued post-specialist professional development.

Desirable

  • Extended or Independent non-medical Prescriber (V300)
  • Physical assessment course or similar.

Experience

Essential

  • 1 years' experience in specialist area
  • Knowledge of, and ability to interpret and apply all relevant trust and professional policies, procedures and guidelines Knowledge of processes involved in health and social policy development and of current policies and their impact on community nursing services.
  • Knowledge of the physical, social, emotional, psychological, mental, cultural and societal effects of ageing, disability, and chronic ill health, and ability to assess an individual's needs and behaviours and plan care within this broad context.
  • Autonomous practitioner

  • Facilitating meetings and groups research projects or involvement within research/audit
  • Experience in community/primary care setting

  • Able to demonstrate highly developed clinical practice using fine motor dexterity and mental accuracy to undertake and adjust treatments.
