Urgent Care services across Herefordshire & Worcestershire, ensuring
holistic and patient focused care is delivered as close to home as possible?
Following the publication of the 10yr Plan this is an exciting time to join the
team, promoting least restrictive alternatives and continuing to develop
integrated pathways across all systems and partnerships to improve patient
outcomes.
The portfolio will
cover the Worcestershire based Crisis Resolution Team and the Crisis and Home
Treatment and Mental Health Liaison Teams in Herefordshire. A demonstrable recent management and clinical
background in urgent care is essential as we seek to further embed these
services and build on the already strong links within the Service Delivery Unit
and wider Trust.
Main duties of the job
The Crisis Resolution Team is a 24/7 nurse led service which provides
assessment, signposting, and brief interventions to people within
Worcestershire County. They accept referrals from several agencies and GP
practices. In addition, the team manage the 24/7 MH Helpline and work closely
with the West Mercia Police and West Midlands Ambulance Service to ensure that
people can access appropriate support in a crisis.
The Crisis and Home Treatment Team in Herefordshire provide an early
referral service for community teams to support patients. The CAHTT work
closely with the adult inpatient wards to identify of patients who can continue
their treatment at home and facilitate an early discharge. The team also
provide an out-of-hours Emergency Department response to the County
Hospital.
The Hereford Mental Health Liaison Team provide specialist mental health
input to the County Hospital (WVT). The Service provides an interface between
mental health services and the general hospital, which is accessible and
responsive. It is a single point of access for people coming into the Emergency
Departments (ED) or who are inpatients on the general hospital wards.
