The Mental Health & Learning Disability Services on the Isle of Wight have been transforming services over recent years to ensure we provide services that will meet the needs of the island population. We have a strategy to provide a 'No Wrong Door Service' whereby minimising the need for people who come to the MHLD service attention to repeat information and be seen by numerous services.
In order to ensure Acute Mental Health services support people through the No Wrong Door strategy, we have been transforming the current crisis pathway to a single Crisis Resolution Home Treatment Team which involves expanding the scope and capabilities of the current Home Treatment Team.
The CRHT team is seen as a vital mental health service dedicated to delivering high-quality care and support to individuals experiencing mental health crisis. We are looking for people who share our vision of our CRHT service playing a pivotal role in providing timely and effective interventions, ensuring that individuals receive the care they need in the least restrictive setting possible.

Job Description: As a member of our Crisis Resolution and Home Treatment Team, you will be providing support to service users in crisis as an alternative to hospital admission. The role will include supporting the wider multidisciplinary team to complete assessments, providing therapeutic interventions through development of long term social and practical support, and running of groups. There will also be a requirement to complete admin generating tasks and communicating with allied organisations.
Key Responsibilities:
- To positively promote the recovery and independent living of service users within the community, taking into consideration holistic needs (education, welfare, financial, leisure, housing, spiritual, etc.), in partnership with other individuals and agencies.
- To adhere to recovery principles and therefore support and enable service to regain control, responsibility, hope and enjoyment.
- To support service users to engage with their individual care plan, working in collaboration with both service user and their family or carer, if appropriate.
- To provide support and therapeutic intervention time to service users in order to promote their recovery in their community environment, helping them to build their own support network.
- To provide support to an identified group of patients in order to assist in the assessment of their mental state and inpatient admission.
- To support with admin tasks identified through the assessment and discharge process as directed by the Senior Practitioners.
- To liaise with allied agencies to include health, social and third sector to ensure on-going service user needs are coordinated in an integrated manner.
- To have the individual service users needs at the fore at all times and use agreed values and skills to underpin their ongoing journey.
- To assist the mental health practitioners in administering agreed standards of care to all service users.
- To maintain accurate records as required by existing policy entering appropriate details on the service users case notes as required.
- Support service users with their social needs, encouraging them to access relevant agencies according to their needs.
- To provide information to patients being discharged to provide them with ongoing support.
