Community Mental Health Nurses for the New Intensive Assertive Community Treatment Team for Psychosis

The Intensive Assertive Community Treatment Pathway is a new pathway sitting within our existing CMHT's. CWPT have secured funding to provide intensive treatment for people with psychosis who have risks linked to non-engagement. This is following the recommendations from the Enquiry into the Incident in Nottingham in 2023.
The service will be multi-disciplinary, consisting of OT, Nursing, Psychiatry, Psychology, Social Worker, VCSE, dual diagnosis workers, support workers and peer workers. The service works closely with health, social care, statutory and voluntary sector partners.
The focus of the pathway is to provide intensive evidence based interventions to some of the most vulnerable in society. The team will be truly collaborative to work with this challenging but rewarding client group. We are looking for enthusiastic, dynamic and experienced individuals who can work confidently on their own as well as being a team player and have knowledge and ideas on how to assertively engage. Good communication and problem-solving skills are essential, as well as knowledge of the needs of people with psychosis. We also are looking for people who are motivated to be involved in innovation and service development.
Main duties of the job
You will keywork no more than 15 patients and provide nursing professional specific support to the rest of the team. The pathway team will operate a shared caseload in their specific geographical / specialist area and will have knowledge of the entire caseload to provide truly collaborative continuous care.
There will be the expectation to complete training and deliver group and individual interventions / specific specialists roles within the pathway for example ward / police liaison as per the national recommendations for this pathway.
