Job summary

RUH Hospital at Home provides intensive hospital level care for acute conditions that would normally require an acute hospital bed, in a patient's home, for a short episode through a multidisciplinary healthcare team. Evidence and experience points to various drivers for developing a Hospital at Home service for older people; key is the provision of a better person-centered care experience, which avoids the risks of healthcare acquired infection, and/or institutionalisation. The RUH Hospital at Home team provides short-term intervention and continuation of medical recovery and rehabilitation at home. The aim of the RUH Hospital at Home team is to provide hospital standard care in the person's own home to maintain the independence and dignity of our patients.

Main duties of the job

  • Assess, differentially diagnose, plan, implement and evaluate individual treatment interventions for patients by virtue of expert clinical judgement and close liaison with members of the RUH Hospital at Home MDT.

  • Use expertise and decision-making skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses.

  • Exercise professional judgement to manage risk appropriately, especially where there may be complex and unpredictable events and supporting teams to do likewise to ensure safety of individuals, families and carers.

  • . Role model the trust values, demonstrating a person-centered approach to service delivery and development.

  • . Evaluate their own practice, and participate in multi-disciplinary service and team evaluation, demonstrating the impact of advanced clinical practice on service function and effectiveness, and quality
