Job summary

The Health Visiting service is part of the wider Children's Health services which provides Community based services for children and young people (aged 0-5 years). The service works with stakeholders across a number of settings and organisation's to lead delivery of the Healthy Child Programme to improve child health outcomes and ensure that families at risk are identified at the earliest opportunity.

The post holder as an effective member of the service will support the delivery of the Healthy Child Programme (HCP) 0-5 to a defined population within the 0-5 thriving Hub. Using evidence based and needs led service; the post holder will offer packages of care in a variety of community settings, which include child health clinic, home, early year's settings and children's centres.

Main duties of the job

Work within scope of competence and practice, conducting health assessments and developmental screenings for children, supporting children to reach their potential and good levels of development.

Provide early intervention and support for developmental concerns.

Providing advice and support to parents on child health, nutrition, and development.

Implementing care plans for children with identified needs as part of the Family Help approach.

Assist in the planning, delivery and evaluation of programmes of care which aim to improve physical and emotional health and wellbeing such as obesity/healthy eating, parenting programmes, children's & parental emotional health, health education & promotion activities. These programmes might be delivered to individuals or in groups.

Support qualified professionals delegated duties to assess the needs of children and families through individual, health reviews, supporting assessment of emotional health needs, monitoring of growth and obtaining the voice of the child

Leading and coordinating health promotion activities within the nursery setting, as agreed by the team.

Supporting the nursery staff with training and development on health-related topics.

Liaising with other health professionals, such as GPs, paediatricians, and social workers, to ensure integrated care.

Maintaining accurate and up-to-date records of all interactions and interventions.

  • NCFE CACHE Level 3 Diploma in Child Care and Education/ BTEC National Diploma in Children's Care, Learning and Development/NVQ Level 3 in Children's Care, Learning and Development
  • Foundation Degree or evidence post qualification education and/or development

Knowledge & Expereince

Essential

  • Experience of working with a Healthy Child Programme (e.g. School Nursing/health visiting) or community settings
