Band 6 Community Diabetes Nurse
An exciting opportunity has arisen for a Band 6 Community Diabetes Nurse to work with our I Diabetes Nursing service in the borough of Redbridge and the wider BHR teams.
We are looking for an enthusiastic and motivated individual, with excellent communication skills, who is able to work autonomously, as part of the team and to be an integral member of the wider Diabetes teams in supporting people managing the complexities of diabetes management.
Up to date knowledge of Diabetes management and drug therapies is essential
Previous experience of working in a Diabetes team and Community Nursing experience are desirable.
The ability to travel effectively across the boroughs is necessary.
Main duties of the job
This role requires facilitating patient structured education , having a sound knowledge of the principles of diabetes management and care and a keen interest in professional development & advancing with technology. Therefore experience of teaching and good IT skills is essential.
The post holder will be responsible for providing; supporting and developing the delivery of high quality; evidence based Diabetes clinical practice throughout the local community, in health centre clinics and patients' homes. The successful applicant will be required to work closely with the lead Diabetes consultant, GPs, nursing colleagues and other members of the multidisciplinary team.

The induction will be held at our head office in Rainham, Essex.High Cost Area Supplement - LondonThis post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum pro rata).
