Job summary
The Paediatric Diabetes Specialist Nurse (PDSN) Lead is a post with a clear focus on improving the care of Children and Young People (CYP) and to support them and their families to manage their diabetes from diagnosis through the transition to adult services.
In conjunction with other members of the Paediatric diabetes multi-disciplinary team, co-ordinate care for children and young people with diabetes and their families, assessing, developing, implementing, and evaluating programmes of holistic care and providing specialist clinical advice to health care and others.
Uses specialist clinical judgement and autonomous decision-making concerning diabetes care and management of CYP. Autonomous decision making to include blood glucose data review and titration of insulin doses in accordance with local policy. The post holder will work in collaboration with medical and other multi-disciplinary staff to provide a comprehensive service focused on diabetic children and young people and participate in teaching programmes to parents/carers, children, young people and the direct staff working in nurseries and schools with this group of children.
The post holder will work as part of the multi-disciplinary team which provides diabetes services to children, young people and their families within the hospital and community settings.
The post holder will hold line management responsibility for PDSNs within the team and appropriately escalate any concerns/requirements.
