Job summary

Mid Yorkshire Urgent Treatment Centre has an exciting opportunity for an Advanced Practitioner who has urgent and primary care experience to join a dynamic and diverse multi-professional team.

The primary aim of the role is to ensure that a high quality urgent and primary care service is delivered to patients attending the Urgent Treatment centre in a timely manner. Our vision at the UTC is to deliver excellent service for patients attending with urgent and primary care conditions.

We are looking for an enthusiastic and self-motivated autonomous Advance Practitioner who has completed the Independent Prescribing module at Level 7 and has registered this qualification with their relevant professional body.

Main duties of the job

The Urgent and Primary Care Advanced Practitioner will act as an autonomous, accountable practitioner working on their own initiative and in collaboration with nursing, medical and allied health professionals. To assess individual patients who present with urgent and primary care condition, initiate investigations, determine a differential diagnosis and initiate appropriate holistic evidence-based treatment and care.

The Advanced Practitioner will:

Manage their own caseload of patients who present with urgent and primary care undifferentiated and undiagnosed illness.

Undertake audit within the clinical setting to develop an evidence base.

Provide formal and informal teaching, support and clinical supervision to nursing staff and junior medical staff.

Provide support to more junior members of the advanced practitioner team and the wider clinicians within the urgent treatment centre.

Ensure maintenance of clinical excellence.

Support the wider multidisciplinary team, undertaking independent comprehensive assessment and management of patients to incorporate physiological, psychological and pharmacological considerations, making clinical decisions regarding patient management and clinical outcomes, including, where appropriate, the discharging patients from the service when treatment is complete/optimum outcome is achieved.

  • To be competent in the assessment and treatment of patients presenting with a range of urgent and primary care undifferentiated and undiagnosed illnesses underpinned by extensive knowledge of a range of chronic primary care.
  • To be competent in providing an initial clinical diagnosis and in the decision to refer to an appropriate speciality.
