Senior Physiotherapist - Ageing well
As an experienced Physiotherapist with post graduate experience in rehabilitation and working as part of a community multidisciplinary team you will find yourself ideally positioned to join our team.
Your clinical skills, expertise and knowledge will enable you to support our urgent care response, rehabilitation, and community virtual ward pathways. You will work within the multi-disciplinary integrated team to jointly assess and plan the care of people who are referred to our ageing well rehabilitation services. The team you will join includes Geriatric Consultants, Advanced Nurse Practitioners, Clinical Pharmacist, Senior Nurses, physiotherapists, Senior Multitherapy Assistants, and close liaison with District Nursing, Community Matrons, Older Peoples Mental Health teams, BHFT Community Wards, Specialist services and voluntary services. You will hold responsibility for the provision of education and clinical occupational therapy advice to staff within the ageing well services.
Main duties of the job
Assist in the supervision and training of physiotherapist students, Senior Multitherapy assistants and be involved in teaching of multidisciplinary staff.
Provide a high standard of physiotherapy for adults with a wide range of physical conditions, including respiratory, orthopaedic, musculoskeletal, neurological, falls, multiple pathology, social issues, and mental health conditions referred to the service. This service is provided in client's homes, residential homes, nursing homes, residential rehabilitation units.
Undertake the comprehensive physical and functional assessment of patients, including those with complex presentation involving physical and mental health components. Use investigative and analytical skills to formulate individualised management plans such as assessing for equipment, assessing transfers and for POC using expert clinical reasoning, utilising a wide range of treatment skills and options to formulate a specialised programme of care in agreement with the patient and/or carer.

- Qualified physiotherapist along with HCPC registration
- Post graduate experience of working as part of a community multidisciplinary team
- Post graduate experience in rehabilitation
- Car owner/driver and possess full UK driver's licence or evidence of ability to travel to designated base and other sites.
