You will be required to cover the 7 day rota which will include; days, evenings, nights, weekends and bank holidays on a rotational basis.

An exciting opportunity has arisen for the role of a Progress Chaser in the Emergency Department based at Worcester Hospital.

The purpose of the Progress Chaser's role is to ensure that patient pathways are effectively managed across site and within Emergency Access Standards.

We are looking for a motivated and enthusiastic individual to join our dynamic team to contribute to high quality patient experience. You will play a pivotal role in supporting the Nurse in Charge and Matron to support patient pathways from admission to discharge/ transfer.

We are looking for a proactive, caring and compassionate person who would like to make a difference to our complex patient group.

Main duties of the job

You'll be joining a team of 4 other Progress Chasers whose role it is to help facilitate the smooth and timely flow of patients through the department and reduce delays, aiding safe and effective discharges and transfers.

Based at the nurse's station in the middle of the department, you'll assist the nursing staff and other health care professionals by tracking each patient throughout their journey through ED to ensure that we are able to meet our targets, whilst delivering safe patient care.

To be sufficiently skilled to understand the ED Key Performance Indicators, including the criteria for four hour targets and breaches, and ensure the reason for the breaches are clearly and accurately communicated and recorded. You will act as a point of contact to support compliance with these essential performance targets.

Job Purpose

To assist the A&E co-ordinator in managing the patients through the admissions / discharge procedure to ensure a smooth journey for the patient within the Emergency Access Target.

To develop and maintain clear and effective communication between operational team / wards / departments and A&E.

Key / General Duties

To adhere to timely escalations as described in the Progress Chaser timeline of actions.

To adhere to timely data entry as described in the Progress Chaser timeline of actions.

To actively chase interventions required to progress patients waiting over 2 hours to ensure Key Performance Indicators are met.

To liaise with specialities to ensure reviews are undertaken in a timely manner.

To liaise with Radiology to ensure scans are prioritised and undertaken in a timely manner.

To liaise with Pathology labs to ensure blood samples are prioritised and undertaken in a timely manner.

To liaise with transport service to ensure timely collect of patients from A&E.

To liaise with the discharge lounge to ensure timely discharge / transfer of patients from A&E.

To record all escalations on Sunrise staff messages.

To enter breach reason on Sunrise before or on 4 hours.

To ensure patient notes are readily available upon request

To assist in co-ordinating all inpatient transfers.

To ensure the nurses stations are tidy and well organised at all times.

To advise relevant medical / surgical specialty of GP arrivals.

To request portering service for patients to be relocated to the wards.

To work collaboratively with all disciplines from across the Trust, to ensure that Key Performance Indicators are met.

To carry out any other duties as requested.

Knowledge, Skills & Experience Required

Excellent communication skills - verbal and written.

Computer literacy essential requirement.

Ability to work as a team member essential with all disciplines.

Previous experience of Medical Terminology and Business Administration qualification (NVQ) - at least two years' experience.

Excellent administration, organisational and time management skills and the ability to use own initiative, work under pressure and have a polite and helpful manner.
