An exciting opportunity has arisen for you to join the NE Essex Community Dementia Support Team. We are seeking an enthusiastic Social Worker who has a caring and compassionate approach to their work. The post holder will be expected to provide support to adults under the age of 65 with a diagnosis of dementia/memory impairment their family & carers. This will involve coordinating care, completion of 117 reviews, discharge planning and care Act reviews as per the Care Act (2014). Alongside, sourcing placements and care packages/personal budgets and supporting the band 4 Social Worker Assistant. This is a unique role whereby you can have a positive impact on the care delivery and support offered to this client group.
Main duties of the job
- Assess, manage, careplan, support and review care needs in line with policy, procedures and legislation, including the provision of aftercare under Section 117 of the Mental Health Act.
- Act as a care coordinator for your own caseload for people under the age of 65 with a diagnosis of dementia/memory impairment within the NE Essex Dementia Service.
- To understand and utilise Health and Social Care commissioned services, demonstrating an understanding of the requirements of the Care Act (2014), and working in collaboration with nursing and other health care colleagues to ensure the appropriate and timely presentation of cases to panel, and the robust reviewing of placements to ensure effective and efficient use of resources, and recovery based delivery of services.

You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7 day a week, 24 hours a day service for 365 days a year working shifts where appropriate.
