Job summary

  • The practitioner will respond to the psychosocial aspects of care relating to children and young people (CYP) with severe obesity, their carers and family members. Home visits will be an important aspect of initial assessments and to follow up needs in the community.
  • The post holder will have knowledge and experience of working with child protection and will function as a skilled and experienced practitioner within the East Midlands CEW Clinics.
  • The post holder will be a visible role model, providing clinical support and practical guidance on all aspects of psychosocial care. Working in partnership with the multi-professional team will be a fundamental requirement to ensure that social aspects of the child's and family's needs are promoted and addressed.
  • To deliver a needs-led, professional social work service to children, young people, adults and their families, complying with legislative requirements and in accordance with established best practice
  • To help maintain a high standard of care within the service by working in partnership with the child, young person and parents/carers and other health care professionals in order to advocate healthy, emotional and practical needs of individual patients and families

Main duties of the job

  • To take responsibility for a caseload of complex cases of CYP with obesity requiring high level, specialist expertise. Support will also be given to carers, siblings and other family members.
  • To enhance the CYP's quality of life within the family and assist them with adapting to a changed life style within the context of the demands of their day to day lives and other commitments and to support them in the transition from children's to adult services.
  • To identify needs, provide and manage psychosocial care for CYP with obesity and their families alongside other mental health professionals/CAMHS worker etc.
  • To promote people's equality, diversity and rights.
  • To liaise and provide specialist knowledge on the social needs of CYP with severe obesity and their families to both statutory and non-statutory agencies both within the hospital and the community, including the child's/young person's educational provision.
  • To undertake the measurement and evaluation of own work/department through the interpretation and application of evidence based practice.
  • To be aware of statutory legislation and practice guidance applicable to the care group and their families and ensure its implementation when planning care.
  • To be an active member of the multi-professional team planning care for the patient and their family.
  • To represent the MDT at multi-professional team meetings, discharge planning meetings, psychosocial and safeguarding meetings as required.
