Job summary
Exciting opportunities have arisen for experienced Healthcare Support Workers to join our friendly and dynamic community teams.
We currently have the below opportunities available:
- A full time (37.5 hours per week) role covering Eastleigh and Romsey which is based at Newton House and operates Monday-Friday 9am-5pm. This is an Older Person's Community Mental Health Team supporting older people with mental health needs, including those with Dementia.
- A part time (30 hours per week) role in the West New Forest area (Ringwood and Fordingbridge) which operates 8am-8pm Monday to Sunday.
These are diverse roles offering responsibility for maintaining patient's health and well-being in their own homes, with the aim of avoiding any unnecessary admissions into Acute Hospital In-Patient setting. Our aim is to provide high quality, safe services which improve the health, wellbeing and independence of the people we care for. Main duties of the job
To manage own workload under the direction and supervision of professionally qualified staff.
To provide physical and psychological care to service users in accordance with the specified plan of care
To observe, report and record changes in the clients physical, mental health and behaviour in accordance with the care plan.
To undertake prescribed physical observations as required in accordance with the care plan.
To work collaboratively with the service user and as appropriate their family, friends and carers, to maintain and or facilitate the achievement of recovery focussed goals.
To be an integrated member of a multi-disciplinary team working in partnership with colleagues from other disciplines, teams and services.
Review of medication for efficacy and adverse side effects
Signposting to appropriate services.
To participate in and where required facilitate therapeutic group work
