Job summary
Sutton health and Care (SHC) Reablement Unit aims to support patients to regain function and independence prior to discharge by providing intensive multi disciplinary rehabilitation on the Reablement Unit.
The activity coordinator will lead on developing an patient centred activity programme, facilitating groups and individual interventions and activities to enhance their quality of life and contribute to their ongoing rehabilitation goals. The post holder will work as a member of the multidisciplinary team, liaising closely with nursing, therapy staff, and reablement assistants to design person centred activities contributing positively to the wellbeing of patients
Main duties of the job
To work closely with the MDT in identifying and facilitating patient centred activities contributing positively to the wellbeing of patients.
Work with members of the muldisciplinary team and ward volunteers to prepare and implement social, leisure and recreational activities with patients
Arrange individual activities for patients as appropriate providing 1-1 social contact with patients according to the individual ability of the patient to participate.
To provide culturally appropriate activities for patients
Ensure there are a range of individual activities for patients; e.g. board games, electronic games, music therapy, creative crafts and reading/observational material.
Organise social events on the Reablement Unit to include patients, relatives, staff and visitors.
Organise and arrange group activities for patients to promote social interaction.
