This is an exciting opportunity to join the Humber and North Yorkshire Dynamic Support Keyworker Service.
This role will focus on supporting autistic young people and those with a learning disability up to the age of 25 who are at the highest risk of being admitted to a mental health hospital. The service supports people across Humber (Hull, East Riding, North Lincolnshire or North-East Lincolnshire), North Yorkshire and York.
We are looking for people with lived experience or experience of working with autistic people and/or people with a learning disability or both.Are you a good communicator and able to work closely with a range of multi-agency partners?

You will be expected to visit individuals in their home environment, setting or community and to work with local services in those areas.
You will need to hold a drivers licence and the ability to commute across Humber and North Yorkshire with access to the appropriate means of transport would be essential.
There will be an expectation to attend face to face team meetings at Humber Teaching NHS Foundation Trust Headquaters at Willerby and this be your contractual base however with an expectation of home working in the first instance. We offer:
Training to new Dynamic Support Keyworkers to ensure that they have detailed knowledge and understanding of processes relevant to the role.
Opportunity to work alongside our existing Dynamic Support Keyworkers to develop good practice, knowledge and new networks with relevant stakeholders and groups.
