This is an exciting opportunity to join the Humber and North Yorkshire Dynamic Support Keyworker Service.

This role will focus on supporting autistic young people and those with a learning disability up to the age of 25 who are at the highest risk of being admitted to a mental health hospital. The service supports people across Humber (Hull, East Riding, North Lincolnshire or North-East Lincolnshire), North Yorkshire and York.

We are looking for people with lived experience or experience of working with autistic people and/or people with a learning disability or both.Are you a good communicator and able to work closely with a range of multi-agency partners?

  • Experience of liaising with multi- disciplinary professionals and working across agencies.
  • Experience of children and adults mental health and human rights

Desirable

  • Have experience and skills in undertaking holistic whole family assessments and risk assessments.

Qualifications

Essential

  • Professional qualification equivalent to degree level in a relevant subject, or equivalent level of experience of autistic people, people with learning disabilities, or both, gained through lived experience, education, youth work, health, or social services.

Desirable

  • Lived Experience specific to learning disabilities, autism or both with children, young people and adults up to the age of 25.

Knowledge

Essential

  • Understanding of a range of work procedures and practices, which require expertise within a specialism or discipline underpinned by theoretical knowledge of relevant practical experience.
  • An understanding of the difficulties children and young people are likely to face with a learning disability, autism or both and complex needs.
  • An understanding of the education, health, and care system (to include those aged 16-25) to fulfil the key working function of holding services to account.
  • An understanding of the Care (Education) and Treatment Review (C(E)TR) and Dynamic Support Register process, and challenges.
