Job summary

The post holder will be employed by London North West University Healthcare NHS Trust (LNWUHT), with operational responsibility held by the Trust and a dotted line of accountability to the North West London (NWL) Integrated Care Board (ICB) Continuing Healthcare (CHC) team. The post is funded by the ICB to support delivery of CHC services across NWL.

This is a community-based post supporting the delivery of high-quality, person-centred NHS Continuing Healthcare assessments for patients discharged from acute hospitals and inpatient rehabilitation units into community settings.

The post holder will undertake comprehensive health needs assessments and CHC eligibility assessments in line with the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care, primarily in patients' own homes or care home settings.

They will work closely with Social Services, ICB CHC teams, community health professionals, hospital discharge teams, GPs, and voluntary sector organisations to ensure safe and timely progression of the CHC process following discharge.

The role will involve supporting complex patient cohorts, including those discharged under gap/complex funding arrangements (e.g. delirium, non-weight bearing, challenging behaviours), and ensuring appropriate reassessment when clinically indicated.

Main duties of the job

  • Undertake screening assessments (Checklist) and full CHC assessments (DST) for patients discharged to the community, in line with the National Framework and local pathways.

  • Work in close partnership with Social Services, ICB CHC teams, community health professionals, GPs, hospital discharge teams, and voluntary sector organisations.

  • Optimise hospital flow and support safe, person-centred discharge by facilitating timely CHC assessment and decision-making.

  • Act as a clinical resource for patients, carers, colleagues, and partner agencies on CHC processes and eligibility.

  • Manage complex patient cohorts, including those discharged under gap/complex funding (e.g. delirium, non-weight bearing, challenging behaviours), ensuring appropriate oversight and reassessment.

  • Contribute to safeguarding processes by identifying and appropriately referring any concerns identified during assessments.

  • Support the wider CHC service through participation in service development, quality improvement, and training activity
