Job summary

The Health Improvement Practitioner will play a pivotal role in delivering evidence-based health promotion activities within the North of Tyne Bowel Cancer Screening Programme (BCSP). Working under the direction of the Programme Manager, the post holder will lead on identifying inequalities in screening uptake and implement targeted initiatives to address them.

This role involves extensive use of patient databases (e.g., ICARUS, PAS, Endovault, E-Record, OBIEE) and requires collaboration with a wide range of internal and external stakeholders across health and social care sectors. The post holder will work across multidisciplinary teams and community networks to promote service accessibility, engage underrepresented populations, and improve overall programme performance.

Main duties of the job

Deliver Health Promotion Activities: Plan and implement evidence-based health improvement and promotional activities aimed at increasing bowel cancer screening uptake across diverse populations.

Tackle Health Inequalities: Identify barriers to screening in underrepresented groups and develop targeted strategies to address inequalities, including service modernisation and community engagement initiatives.

Database and Systems Management: Use and maintain multiple patient-related systems for data input, analysis, and reporting to support service delivery and audit processes.

Public Engagement and Education: Engage with individuals, community groups, and health professionals to raise awareness of the Bowel Cancer Screening Programme. Provide tailored education to promote healthy behaviours and increase screening participation.

Organise and Deliver Events: Plan and lead community outreach and promotional events. Represent the screening programme in public forums, ensuring effective delivery of key messages and materials.

Evaluate and Report on Impact: Monitor the effectiveness of health promotion interventions, analyse data to identify trends or service gaps, and produce reports to inform service planning and improvements.
