Job summary

An exciting opportunity has arisen within the MHSOP Community Mental Health Team for a 12 month fixed term period to operate as a Community Mental Health Nurse within the Older Adult's pathway within a specific Geographical area.To provide specialist skills, knowledge and experience for those identified as having Mental Health problems.To work as part of a Community Team which may be integrated, working collaboratively and in partnership with other services.To be compassionate in meeting the needs of Service User's and their carers.To be professionally accountable and responsible for service user care. To undertake a range of clinical / therapeutic interventions within the service user's home environment or other community settings.

Main duties of the job

Working as a community mental health nurse your role will be to undertake the assessment and management of services users presenting with complex mental health care needs.You will provide a comprehensive assessment of need, offering intervention, advice and support to service users, family, carers and other professionals involved in their care.As part of the multidisciplinary team, you will ensure that care and treatment is delivered to a high quality, ensuring that person centred care is deliver, risks are managed and appropriate treatment and other interventions (pharmacological and non-pharmacological) are offered within a timely manner.You will contribute to the effective care planning of the service user's needs by providing regular reviews, risk assessment with the goal of working towards safe discharge planning.You will practice in accordance with the Trust' Values: Respect, Everyone's Contribution Counts, Responsibility & Accountability, Patients First, Safe & High Quality Care.

Essential

  • Registered Mental Health Nurse Part 1 with Current NMC registration.
  • Successful completion of a post qualification Preceptorship programme.
  • Evidence of qualification to undertake the supervision, teaching and assessing of students (essential requirement for post, expectation this will be completed within an 18 month period unless post is bank only).
  • Evidence of Continuing Professional Development.

Desirable

  • Leadership or management qualification
  • Post-basic qualification at Diploma / Degree leve
  • Non-medical prescribing qualification or interest in working towards.

Experience and knowledge

Essential

  • Relevant post qualification experience in Older Adult Mental Health Services.
  • Relevant post qualification experience in Older Adult Mental Health Services
  • Experience of working in a Mentor capacity with relevant qualification or willing to work towards.

Desirable
