Job summary
An exciting opportunity has arisen to join the Paediatric Diabetes Team as a Patient Care Coordinator. The post holder will work as part of a multidisciplinary team, assisting the Paediatric Diabetes MDT with actions arising from the MDT and Children and Young People's Diabetes Network outcoming discussions. Whilst the post holder will support the Paediatric Diabetes Nurses and also work alongside the clinical Diabetes Team, they will be managed within the Business Unit Administration Team. They will be required to support the wider Child Health administration teams if needed.
Main duties of the job
The post holder will be actively involved in providing indirect care for patients in support of the MDT. They will be expected to work under direct/indirect supervision of the clinical nurse specialists supporting the MDT. They will provide a full and comprehensive admin support and data management to the MDT. They will respond to and act upon phone calls and escalate to the appropriate person, using good communication skills, basic clinical awareness and appropriate tools/procedures.
It is imperative that they have a full understanding of the targets associated with Diabetes, ie Best Practice Tariff and Audit (NPDA). They will be required to communicate with the Children and Young People's North East and Cumbria Network for up-to-date information.
They will assist in the analysis and extrapolation of data through the interrogation of databases and sending secure information to professional bodies. They will abstract clinical and administrative data from casenotes and other agreed sources for the purpose to proactively monitor and track patients through their pathway, ensuring that their journey is smooth and coordinated, ensuring patients adhere to targets set by the Network, tariff and the Diabetes MDT.
They will take the lead in coordinating the transition of young people between the Children's Diabetes Service and the Young Adult Diabetes Servicek, identifying patients at the beginning of the transition pathway and ensuring all transition targets are met.
