Job summary

We would like to announce a great opportunity to work as a Care Navigator within the Healthy Together Helpline.

The Care Navigator will support the Healthy Together Helpline practitioners with liaising with multiagency partners, preparation of records for safeguarding meetings and supporting families with a range of health enquiries. The Care Navigator will be a point of contact for Social Workers when collating non-clinical health information.

The Care Navigator will support with promoting the Healthy Together Helpline at stakeholder events across Leicester, Leicestershire and Rutland.

You must have access to your own transport or suitable alternative. Dependence on public transport is not suitable for this role.

Please note, the base is currently Bridge Park Plaza but will be moving to County Hall this year. Interviews will be held on Tuesday 16th April.

Main duties of the job

The Care Navigator role is specifically to improve outcomes for children, young people and their families and carers. The Care Navigator is a member of the administrative team who supports the clinicians in the team, this involves working closely with the Lead Professionals which include School Nurses and Health Visitor teams who are the universal point of contact for every child.

The service is aimed at families where a child or young person needs additional intervention and information about local support networks. The Care Navigator will focus on health needs specifically relating to the child or young person when they have additional needs or may have social care involvement.

Care Navigators are a recognised point of contact, support child, young people and families to co-ordinate services and partner agencies involvement in case reviews, enabling the families to get the right support from the right professional at the right time. The Care Navigator will arrange and facilitate this where appropriate.

The Care Navigator will be a key member of the Healthy Together Helpline and will utilise the Mittel telephone system, dealing with internal and external calls.

  • 3.5 Knowledge of electronic and hard copy organisation systems and able to use a range of Microsoft Office packages including Microsoft outlook and ability to produce documents presentations, spread sheets and work with databases as required.
  • 3.14 Knowledge of Child Protection and Looked after Children policies and procedures.
  • 5.5 Sensitivity in dealing with parents/carers of children with complex
