The post holder will be part of the Haywood Hospital Therapies Portfolio providing Physiotherapy within the D2A bed based services, Pathway 2 ,
This is a dynamic role that requires collaborative working with our colleagues across the D2A pathway & teams/agencies across the ICS's providing rehabilitation. The role supports specialist rehabilitation managing timely discharges, effective bed flow & supporting adaptation and resettlement back into the community. The overall aim being to improve the health and wellbeing of the population served.
Main duties of the job
The successful applicant will take part in both local, regional and national initiatives. The nature of the service will be determined by local service specifications.
You will
- Be knowledgeable of the D2A model
- Be skilled in the assessment of older adults & support needs and in the development, implementation and evaluation of care/support and rehabilitation programmes.
- Have supervisory, training and leadership responsibilities where appropriate and will have some responsibility for team performance and service developments/improvements.
- Demonstrate initiative and be creative in finding solutions to problems.
- Work with the team leader undertaking delegated tasks in team management.
