Job summary

We in the Community Complex Care Team pride ourselves on our delivery of excellent, holistic patient and family centred care within the home, school and a variety of community settings. All of our client group (Adults and Children) have highly complex physical health needs, which we meet in order to support their independence and enable them to lead fulfilling lives at home with their families.

We are a large, expanding team covering the whole of Staffordshire, with office bases in Stafford, Stoke-On-Trent & Lichfield. We operate in a Hub system, which is, based on geographical location. This allows us to ensure a supportive, small team feel across all hubs. We provide a 24/7 nurse on call service, ensuring a constant point of contact where support can be accessed.

We are recruiting to the following locations:

Stoke on Trent,

Stafford, Uttoxeter and Stone,

Lichfield, Burton and Tamworth.

You will be able to discuss your preferred location during the interview.

This vacancy is ideally looking to recruit a Band 4 Complex Care Support Worker, however, we are also in a great position to be able to offer Band 3 developmental Band 4 posts to candidates who require some additional training and development to help you reach what is required as a band 4.

Main duties of the job

We are looking for Complex Care Support Workers to join our team to help us to support our families. The needs of our client group varies widely, from needing support with their breathing to managing challenging seizure activity and everything in between! We do cover a 24 hour period so there is an expectation that you would be able to work days and nights and the shift times are individual to each package of care. The whole of your shift will be spent with one client, so you will get the opportunity to really make a difference.

Full training will be given to meet the individual needs of the client but previous care experience is required.

  • To work in patients homes or other designated settings to deliver high quality care to patients with complex physical health care needs, including but not limited to management of tracheostomies, ventilators, enteral feeding, complex seizures and medication administration.
  • To assess patient care needs on a daily basis, implementing and evaluating care plans produced by qualified nurses with patients and their families.
  • To assess care plans, risk assessments and contingencies, in conjunction with the qualified nurses, on a regular basis to ensure consistency and reflect up to date care needs.
  • To ensure the delivery and quality of care given complies with local and Trust agreed policies and procedures and is of the highest standard.
  • To assist patient in achieving their potential to live independently as per care plan.

Qualifications and training

NVQ Level 3 in related subject/ Nursery nurse qualification or similar

Experience

Significant experience in similar role with demonstrable commitment to mandatory training, attendance and engagement.

Previous care experience in a health, social care or childcare setting.

Experience of communicating highly complex or sensitive information and using skills to overcome barriers to understanding, for example with non-verbal patients

Experience dealing with complex care needs, including management of tracheostomies, ventilators, enteral feeding, complex seizures and medication administration

Understanding of Information Governance, data Protection, Care Certificate HCPC/NMC standards, and safeguarding vulnerable children and adults

Knowledge

Excellent communication and writing skills.

Ability to travel across Staffordshire.

Able to work on own initiative without direct supervision.

Ability to maintain professional boundaries/ diplomacy whilst working in intense conditions.

Able to work well within a team.

Able to maintain confidentiality.

Able to recognise own limitations and scope of practice.

Knowledge of and willingness to use digital systems.
