• You will need to be willing to travel within the Burton and Uttoxeter areas, as the successful candidate will work between both areas.
• You will be a registered nurse with a keen interest in working with children.
• You will be responsible for a defined population of service users in a variety of settings across the Trust including the community
• You will be skilled in the assessment of care and support needs and in the development, implementation and evaluation of care and support packages.
• You will have supervisory responsibilities as required (e.g. work allocation) and may have some responsibility for service developments.
• You will deliver delegated aspects of the 0-19 Healthy Child Programme, ensuring key objectives and outcomes are met
• You will have the ability to use and undertake training regarding electronic based IT platforms including electronic patient records, including Rio and in the future the E- Red Book, Patient Knows
• The post may at times require working between several health clinics in a local area as per needs of the service.

The post holder will be part of the 0-19 Families Health and Wellbeing Service providing public health advice, information, guidance, and referral and providing early intervention. In doing so, they will contribute to the assessment, planning, delivery and evaluation of programmes of health, ensuring that they practice within their sphere of competence and knowledge.

They will work to professional codes, standards and guidance at all times ensuring that their practice is grounded in evidence based theoretical and practical knowledge.

The post holder will promote the health of children aged 0-19 within the Staffordshire area through the public health agenda, thus enabling each child to reach his/her development, and educational potential.

This is an exciting opportunity that requires the Staff Nurse to work closely with multi-disciplinary teams in addressing a range of care and support issues across a spectrum of needs and preventing unnecessary hospital admissions. They will contribute to the delivery of the Healthy Child Programme (0-19); ensuring key objectives and outcomes are achieved and to the improvement of the health and wellbeing of the population served. The nature of the service will be determined by local service specifications.

To adhere to and apply the Nursing and Midwifery Council (NMC) Code of Conduct and all other relevant guidance from the, Nursing & Midwifery Council (NMC) and the Trust. To be professionally accountable and responsible for all aspects of own work.

To establish and maintain effective working relationships both within the Partnership Trust and with external organisations in order to meet service requirements and support and demonstrate the Trusts equality values.

To assess children, young people, families and carers, care and /support needs. Formulates, develop, implement and evaluate programmes of care and support. The care and support plans may include the following:

• Promoting independence/self-management and maintaining safety
• Increased functional ability
• Improvement in health and wellbeing including mental health
• Interventions in disease/condition management
• Prevention and reducing of health inequalities
• Admission avoidance
• Early supported discharge
• Case management/ treatment/ care plans
• Palliative and end of life care
• To weigh and measure children, young people and infants and record accurately
• To support the public health practitioners with the completion of caseload profiles identifying health needs
• Health promotion, advice and guidance
• Signposting to other services.
• Contribute to the health profiling information and local data in line with key performance indicators.

To use professional knowledge and judgement to take into account the range of care/support packages and treatment options available. To consider the personal circumstances and preferences of children, young people and families in order to implement and adjust programmes of care and intervention.

To maintain and develop skills and knowledge to deliver a range of interventions and set an example of good practice to other team members including skill sharing.

To maintain sensitivity at all times to the emotional needs of children, young people, families & carers offering reassurance and empathy. This may be in regard to the nature/prognosis their condition/situation and implications of the same.

To work with individuals and carers who may find it difficult to engage with the service or aspects of the service resulting in uncooperative or challenging behaviour that has to be managed.

To use professional knowledge, skills and experience when working in changing environments and across professional boundaries.

To adhere to Staffordshire Safeguarding Children Boards guidelines, to identify and report any case of suspected or confirmed abuse. To support any children and their families with identified health needs within the Child Protection and Looked After system, contributing to reports in accordance with Child Protection policies and procedures.

To maintain contemporaneous and accurate records as required by the Trust and in line with professional standards (e.g. /NMC). This includes self-management and supervision of identified team members, giving guidance and direction where appropriate.

To take responsibility for the safe use and transportation of equipment between locations. To work in non-purpose built rooms involving the necessity to move furniture, therapy equipment etc.

To ensure that the skills of identified team members are appropriately deployed and that care is delivered by staff with the relevant education, experience and competence.

To deliver a high standard of intervention, whilst managing the demands of unplanned aspects of the role using innovative problem solving techniques where appropriate.

To provide advice to service users, carers and professionals regarding the management and care and support of children, young people, families and carers for example regarding continence and enuresis.

To provide professional and clinical support and leadership to identified team members.

To take accountability for allocated assessments, planning and implementation of care packages and programmes of support.

To provide and receive regular supervision in order to support the continuous professional development of self and others.

To contribute to the process of induction for new team members and other disciplines, as appropriate.

To identify own training/educational needs as part of the supervision and appraisal process. Attains and maintains an agreed level of expertise through ongoing training and development.

To have an understanding of people management skills which incorporate compassionate and appreciative leadership.

To set workload priorities for self, and oversee the workload of team members and students as required.

To provide training, undertake work based assessments and supervision of pre- registration students, typically for periods of 4-12 weeks as required.

To participate in and support the team in undertaking clinical audit.

To contribute to, and actively promote the implementation, development and upholding of policies, procedures, guidelines and directives, along with relevant statutory measures and governmental directives.

Qualifications

Essential

• Level one Registration with NMC.

Desirable

• Degree or diploma in nursing

Training required

Essential

• Evidence of/willingness to undertake post- registration training relevant to the post.
• Experience working with children/young people.

Desirable

• Perinatal mental health Safeguarding children levels 1, 2 & 3 Solihull Training Child and adolescent emotional and mental health awareness/training. Obesity training ERIC training

Work Experience

Essential

• Relevant experience/placements to the role including assessment, care planning and, management to support of children, young people, families and carers. Ability to manage time and caseload effectively. Working in a multidisciplinary team.

Desirable

• Experience in working with children and young people in the community. Customer care, health improvement and previous community working. Relevant experience/placements to the role including child development and behaviour management. Experience of running children's evidence based enuresis/continence clinics.

Knowledge

Essential

• Evidence of clinical knowledge relevant to the post. Healthy Child Programme 0-19, High Impact Areas (Early Years and School Aged Years) Understanding of Information Governance, Data Protection, NMC standards and safeguarding children.

Desirable

• Awareness of standardised assessments and outcome measures. For example ASQ / ASQ SE Awareness of relevant NSF's and NICE guidance Children's safeguarding levels 1, 2 & 3
