SIM (Specialty Integrated Medicine) at LTHT we have an exciting chance for a
Band 6 role within one of the Older Adult admission wards for a 12 month
secondement. Ward J21 is currently a
30 bedded assessment area, accommodating a combination of female and male
patients. The post holder will be
responsible for leading the ward staff and working closely in collaboration
with the MDT across Specialty Integrated Medicine.
J21
provides a great standard of quality care for all our patients and we are
looking for a highly skilled nurse to continue the fantastic work that J21
has already achieved in caring for our older population in Leeds. We are
looking for a enthusiastic and dynamic individual with the capability to
empower a team whilst remaining patient centered.
Applicants
will be expected to practice effectively & deliver excellent care in a
timely manner to our patient group.
A
commitment to personal professional development is essential with the promise

Candidates
are required to have a least 12 months experience at Band 5 level and have
substantial acute nursing background with experience in caring for patients
in an acute setting.
Main duties of the job
Acts
in accordance with the Nursing & Midwifery Council (NMC) Code of Conduct.
Communicate
with patients and carers to ensure they have access to appropriate
information.
Maintain
and deliver compassionate patient focused care in accordance with LTHT
guidelines, policies, values and behaviours.
Provide
assessment, planning, implementation and evaluation of nursing care for
patients, ensuring documentation standards are upheld in accordance with NMC
and LTHT guidelines, whether written or computerised.
Promote
and maintain positive relationships between all staff involved in the care of
patients.
Promote
evidence based practice.
Participate
in the audit and evaluation of clinical practice in the interests of
promoting and improving quality care through action on findings.
Undertake
duties on other wards or departments as and when required by service demands
or patient need.
Act
in such a way that safeguards the health and wellbeing of children and
vulnerable adults at all times. Is familiar with and adheres to, the LTHT
safeguarding policies.
Contribute
to the promotion of a positive clinical learning environment.
Keep
up to date with developments, advances and research in individualised patient
care.
Participate
in research and or education programmes relating to professional nursing
issues and practice.
