Qualifications
Registered Nurse (Level 1).
Evidence of theoretical, educational and clinical development related to the
field of critical care.
Recognised teaching/assessing qualification.
National Outreach Framework Enhanced Competenciesachieved or in progress.
Educated to post graduate degree level or equivalent experience.
Advanced Life Support provider.
It is desirable that the post holder, hold a non-medical
prescribingqualification. Following attainment, the post holder will
prescribe within their scope of practice for the management of the
deteriorating adult.Experience
Significant post registration experience.
Significant Critical Care Outreach experience at sister/charge nurse level.
Working with a wide range of professionals including medical, nursing and
management colleagues.
Skills
Ability to utilise expert/high level communication and interpersonal skills
effectively within a multi-disciplinary team.
Ability to teach, supervise and assess qualified and unqualified staff.
Ability to manage time & prioritise own workload effectively. Proactive

Evidence of managing complex patients in stressful and emotional situations
where life-changing events are being communicated and clinically managed.
Knowledge
In-depth knowledge and expert clinical competence in the care of the
deteriorating /acutely ill adult.

Have knowledge of pathophysiology, anatomy and physiology in order to assess,
plan, implement and evaluate treatment of the critically ill patient. Able to
make expert clinical judgements involving complex situations which requires
analysis and interpretation to decide on the best course of action/treatment.
Competently and confidently care for the deteriorating/acutely ill adult
utilising expert critical care skills and knowledge in a variety of clinical
settings with indirect supervision as part of the multi-disciplinary team
Provide rapid response and support for patients identified by the National
Early Warning Score.
Undertake a comprehensive advanced patient assessment based on history taking
and clinical examination and implement a management/treatment plan.
Provide direct patient care and support at the bedside until the patient is
stabilised or transferred to an area with a higher level of care. Initiate
therapeutic interventions for example to manage haemodynamic and respiratory
instability.
Refer to other members of the MDT and other specialities as appropriate.
Participate in the stabilisation and transfer of critically ill patients to
an appropriate environment.
Competent in advanced clinical skills such as venepuncture, cannulation,
arterial stabs, tracheostomy decannulation, airway management and
cardiopulmonary resuscitation.
Accurately request pathology and radiological investigations and make
clinical decisions based upon the results.
Utilise highly developed communication and negotiation skills to deal with
challenging/conflict situations ensuring patient centred care.
Work autonomously taking responsibility for own decision making, referring to
the appropriate physician/ICU Consultant/Outreach Team Leader when advice and
further clinical input is needed or when care and treatment goes beyond
normal service parameters or scope of professional practice.
Carry the crash bleep and respond to 2222 calls. Where required, carry the
critical care bed managers bleep out of
hours,
liaising with the sisters/charge nurses to co-ordinate flow throughout
critical care.
Respect patients rights to participate in decisions about their care and be
able to advise them on complex and personal matters.
Communicate highly sensitive information to patients and their relatives,
showing empathy and providing psychological support.
Maintain up to date clinical records in accordance with the most up-to-date
NMC and Trust guidance.
Act as a clinical role model, which reflects the philosophy, beliefs and
clinical standards of the outreach service.
Maintain a safe working environment. Demonstrate an understanding of key
infection prevention and control policies including, hand hygiene, MRSA,
transfer of patients.
Demonstrate individual responsibility for infection prevention and control,
challenging non-compliance with policy.
Develop and implement evidence-based standards, policies and guidelines.
Act in such a way that safeguards the health and wellbeing of children and
vulnerable adults at all times. Is familiar with and
adheres
to, the LTHT safeguarding policies.
Education
and Training
Contribute to the promotion of a positive learning environment within the
clinical areas across the Trust.
Promote evidence based practice wherever possible. Inform, consult and
educate members of the multi professional team regarding the purpose and
function of outreach and the function and use of the Early Warning Scoring
system.
Participate in critical care based education programmes that are set up
across the Trust and the University.
Lead and participate in identified education/training needs Trust wide for
the deteriorating adult.
Deliver and participate in education programmes across the trust to enable
ward staff to care for the deteriorating /acutely unwell adult.
Participate in preceptorship and mentorship programmes. Undertake
continuous professional and clinical development, seeking opportunities to
enhance skills.
Continue to develop specialist advanced clinical skills in response to
service need.
Maintain annual mandatory training requirements. Keep up-to-date with
developments, advancements within critical care by working clinically in the
critical care environment.
Actively participate in the development of own personal development plan and
performance review.
Work in collaboration with ward managers and clinical educators to identify
the learning needs of ward based staff trust wide
Management
and Leadership
Develop inter-professional collaborative working relationships with staff
trust-wide utilising excellent interpersonal skills.
Use expert/advanced communication skills to create and sustain new alliances.
Investigate and respond to accidents, complaints, untoward incidents and
other significant events.
Provide leadership, support and direction for junior members of staff.
Motivate, inspire and develop other practitioners to achieve excellence in
care.
Provide leadership for the multidisciplinary team as required when managing
acute situations. Act as a positive role model.
Raise concerns with regards to risk, danger, malpractice or wrong doing by
following the LTHT Freedom to Speak Up Policy and supporting processes.
Research
and Audit Maintain up-to-date knowledge of current research/audit and best
practice.
Participate in regular audit activity specific to the client group
as
agreed with the team lead.
