Great Ormond Street Hospital NHS Foundation Trust has an exciting and unique opportunity for a Roald Dahl Complex Medical Care SWAN (Syndromes without a name) Clinical Nurse Specialist in partnership with The Roald Dahl's Marvellous Children's Charity
Main duties of the job
You will be responsible for supporting and developing a collaborative and high-quality service for children and young people (CYP) with medical complexity within the acute children's service. You will support effective communication and develop pathways and processes across boundaries and organisations both locally and regionally
You will focus on improving planning and support for CYP with complex medical conditions, including effective pre-admission, transfer and discharge planning, aiming to reduce hospital stay length and readmission, and promote coordinated, safe and high-quality care
This is a unique 2 year fixed term opportunity to act as an ambassador for the Roald Dahl Marvellous Children's Charity and to be a voluntary representative of the charity when required to represent and promote the role and links between the NHS and RDMCC
There are also opportunities to join and contribute to RDMCC's internal communication platform (Workplace from Meta) with encouragement from the NHS
