Job summary
An exciting opportunity has arisen for a Band 5 RGN/RMN Community Nurse to work within our North Tyneside Recovery Partnership (NTRP). NTRP supports those of all ages affected by substance and/or alcohol misuse.
The post holder will work directly into our Physical Health Pathway completing comprehensive physical health assessments. The role would also entail providing safe and effective interventions, monitoring of health, onward referrals, harm reduction advice and use of psycho social interventions to support service users to consider and achieve their goal.
The team consists of a well-established mix of professionals that work collaboratively to deliver individualised care. The role involves individualised clinical assessment, care planning, formulation and collaborative working across the multi-agency team.
The applicant will be expected to provide supervision to colleagues and support students.
You will work in a highly supported environment and will be provided with training opportunities to further develop your knowledge and skills.
Main duties of the job
To carry a caseload of service users with physical health needs. To promote their engagement and wellness with health services
To offer all service users an annual health check, with a recommendation for any follow up as required.
To assess and take bloods of all service users who are referred with alcohol dependency and require medically assisted detoxification. There will be a expectation of support required throughout the period of the detoxification.
Provision of vaccination including 'flu' and hepatitis B
BBV screening and referral to hepatology/infectious disease centre where required.
To offer ECG's to all service users at risk of prolonged QT interval with ongoing referral where needed.
